Sending a referral to the suite
There is no form to complete. Fax the referral to 306-518-9080 with recent bloodwork and relevant imaging reports. Our office contacts the patient with an appointment time. Mark the cover sheet Uptown Medical Specialists: the fax line is shared with Horizons.
If a referral is urgent, call 306-518-9081 and say so when you fax.
